Early Bird Registration Fee: until March 31, 2011 PHP 10,000.00

Registration Fee: from April 1 to June 30, 2011 PHP 12,000.00

Registration Fee: from July 1 to November 29, 2011 PHP 15,000.00

Registration fee Inclusion Registration fee covers participation, conference materials, daily lunch and coffee/tea breaks for 3 days. It does not include accommodation, which should be booked separately.

Registration Process 1. Please download the registration form from the webpage at: http://www.ncda.gov.ph/

2. Please send the completed registration form to the following address: apcbrcongress@ncda.gov.ph

3. Please check the payment details and find the most convenient option for you. To facilitate payments, participants are kindly requested to pay in Philippine Peso through:

a. PAYPAL, by using an existing account or by simply signing up & creating an account at https://www.paypal.com/ph

b. A manager’s or cashier’s check, payable to the National Council on Disability Affairs, via any courier of your choice. Once your check has been received, the organizer will confirm your registration and send a photocopy of the official receipt.

c. A third option is to deposit payment directly to the bank account in the name of: Treasurer of the Philippines, details provided in the following row. Please fax the deposit slip together with your registration form to apcbrcongress@ncda.gov.ph

d. If it is not possible to use any of these three payment schemes, kindly inform us at apcbrcongress@ncda.gov.ph  on the most convenient way for you to pay the registration fee. It would be much appreciated if payment could be received before the congress.

4. Upon receipt of your payment, the congress organizer will send you a confirmation letter, via e-mail or fax, which will serve as proof of your registration. This should be presented at the registration desk when you check-in at the Congress.

5. Registration will only be officially acknowledged upon receipt of the registration form and fee.
